The simple request is a T by rule and should not be ignored, IMHO

Yes, even if the original try counted, 2 shots. If a 3 pointer was shot and made, still 2 shots (3 if missed as already stated)

I agree. I would only call an intentionaal fould, well at least not call a common foul. A push off trying to get open is much more of basketball play than this.
