You should have enough game awareness to not be surprised by this. You should have an idea when a player has three or four fouls, especially prior to the 4th quarter.
Knowing this won't alter how you call the game, but it will allow you to not mess up anything administratively. You can be more deliberate prior to administering the ensuing throw-in because the coach is likely going to want to substitute for a player with a high foul count early in the contest.
You also don't want to be administering a throw-in or FTs with a player on the floor whom the crew should be disqualifying.
