1: Travel if you were holding the ball (resting in your palm would count as holding it) and exceeded the limits of the pivot foot.

2: Keep the jerseys tucked and this doesn't happen. If it happens on accident, consider it player control of the ball and rule accordingly.

3: If it's done on purpose, it's a T. This ain't the Globe Trotters.
