I don't want this to be something that is a pain to fill out. Maybe just circle 1-5 ratings (poor, below average, average, above average, excellent) and then have a place for comments.

What are some categories I should use?
Scorekeeper:
1. Attentiveness -- i.e. eye contact on foul reporting, NO TEXTING, etc.
2. Accuracy -- Team/Individual fouls and Team Score
3. Neatness -- Accurately and clearly enters players' names/numbers, fouls, score, etc.
4. Communication -- with Timer and Referee Crew
5. Attitude -- being pleasant to coaches and referees, etc.
6. Professionalism -- on time, taking care of "business" at halftime by ensuring timer/clock person agree on score, getting scorebook ready for the 2nd half, etc.
7. Overall Rating

Timer/Scoreboard Operator:
1. Attentiveness -- Starting/Stopping clock on time, Getting time up for the next game warm-up quickly
2. Accuracy -- Team/Individual fouls and Team Scores accurately reflected on the scoreboard,
3. Communication -- with Scorer and Referee Crew
4. Attitude -- being pleasant to coaches and referees, etc.
5. Professionalism -- on time, taking care of "business" at halftime by ensuring scorer and clock agree on score, getting scoreboard ready for the 2nd half by zeroing out team fouls, etc.
6. Overall Rating

Game Administrator:
1. Attentiveness -- Being aware of problems with coaches and/or parents,
2. Treatment of Referees -- water, food, etc.
3. Communication -- with Scorers/Clock Operators, Referee Crews and Coaches,
4. Timeliness of Activities -- Opening of facility, equipment ready for scorers/timers, communication with coaches and referees at halftime and before games as needed,
5. Attitude -- being pleasant to scorers/clock operators, coaches and referees, etc.
6. Professionalism -- on time, managing fans and coaches appropriately,
7. Overall Rating
