I don't want this to be something that is a pain to fill out. Maybe just circle 1-5 ratings (poor, below average, average, above average, excellent) and then have a place for comments.

1) Professionalism
2) Fluid replacement availability (Water)
3) Towels
4) Appropriate Dressing Facility
5) Escort to and from Court
6) 3-Min warning at half-time
7) Correct Attire at table
8) Un-biased attention at table
...these are just a few more specific areas rather than catagories. Others will follow...
