Flagrant, But Not Ejected

Has anyone had a Flagrant Foul that did not meet an ejection??

Fighting = Ejection
Combative Strike/Attempt - Ejection
Spitting/Biting = Ejection

What could account for just a DQ?

Are you discussing something state specific? In NFHS rules there is no such thing as an ejection for a player or substitute. There is only a DQ which is applied for everything from a player fouling out to a flagrant foul for fighting.

Flipping it around, can you have a DQ or ejection WITHOUT a flagrant technical foul (a la Norman Dale's "kick me out of the game" scene)? I don't believe that's possible.

You can also have an EJ of a player (err..team member) -- if someone on the bench is beign disruptive, you can send him / her out of the gym WITH adult supervision.

Can you toss a coach or player without having to shoot free throws (double technicals notwithstanding)?

I have heard stories about officials who have just told a coach to leave because he (the coach) was being an idiot. But I have never done this, nor can I think of rule justification for it.

No. Even the penalty with a direct DQ has a 2-shot component.
