Question What happened to "over the back"?

During the Villanova-St. Mary's game this morning (Pacific time), the "color commentator" said a Villanova player committed an "over the top" foul. OK, what's next - "over the shoulders"?

PS.. I tried to figure the best way to spell man-to-man the way he says it... I came up with these options - mantoman, mntmn, mn-t-mn, mantman... any thoughts?
