1) Agree.
2) Judgment call. You don't want to get into a situation where you nail a player with a run-of-the-mill "T" and some goofus on the other team goes all macho off the bench.
3) One flagrant "T" will suffice imo. You have to do too much explaining on the game report if you nail him with two.
