Might have a rootkit problem... if your scans are coming up clean. I would only recommend a re-format as a last resort. (formatting a pc is such a huge pain)
Run ATF cleaner then TDssKiller then Malwarebytes. just saying. As a technician for our school system.. I have to do this 100's of times a week. (dont get me started...sigh)
