Equipment falling onto the floor is always an issue, eyeglasses (pet peeve, get a strap), pony tail holders, headbands, mouth guards, etc. Many officials will stop the game as soon as possible when any equipment items end up on the floor.

Still, the issue with medic alert bracelets is not falling off. They're designed to not fall off. Being less than almost impossible to come off would effectively defeat their purpose.
