How often is it the referee dropping the ball, and how often the table? I recall seeing this once this season, and it was the table not telling the refs until after the first FT.

This situation happened to me once this season. I reported a foul. The table had a question (I cannot remember what it was but there was a slight delay). When I turned around to see the floor, the first free throw was finished. The table then lets us know that it was the players 5th foul. The coach who's player fouled out wanted a "do over" on the made free throw. I simply told him no and we moved on and no one said a word.
