Honestly, I see this a handful of times a season. Guys get in a hurry and anything outside the routine is a disruptor. Not saying it's right, by any means, but it happens far more than it should.

In fact, I saw it last night in a HS playoff game here. Center gets a good shooting foul, takes it to the table, and warns the home HC after the report for his response. While he is still administering the warning, the first FT went up. Sigh.
