Origin of the Three-Person System?

Does anybody have any reliable historical information as to the origin of the three person system? I had heard that it was developed in Cleveland, Ohio sometime in the seventies, and that the original intent was to provide more complete off-ball observation, but cannot legitimize any of that one way or the other. Does anybody know who came up with the system and about when? Was it a collaborative effort bye a committee or was there one person who developed it?
Thank you in advance for your response if you happen to know anything on this topic.
