The book is clear on when the AP arrow is set -- and it's only when the ball *could remain live* after the action.

Right. What if there's a subsequent technical foul after that thrower is "handed" the ball?

(What, is this the late 1980s? Since when do we hand the ball to a free thrower?)

It's not as long ago as you think. I believe the mechanic changed around 1999-2000.
