Wrong Lineup sheet used

I know its been several years since the lineup changed from the bubble sheets to list form but Tuesday I had a JV coach submit a lineup in the previous form

I told my R2 and the coach tried to pull the "I've been using these for weeks and no one has said a thing" argument with him but it failed. Has anyone else seen a coach try to submit a lineup in the old bubble form?
