Wrong server
Team A's turn to serve. #24 is whistled to serve and coach of Team A trys to call a time out. #24 serves and Team A wins point. Then Team A calls time out. After the time out, #12 then serves for Team A and Team B wins the point. The REF then asks the scorekeeper if #12 was the correct server, which the scorekeeper says that #12 was the correct server but #24 was the wrong server. As the REF, what do you call, if any thing? Do you go back and take the point that team A got when #24 served? Is this a correctable error?

It's correctable until B contacts the ball for their next serve. As this has not happened, and the scorekeeper is certain that there was an illegal server during A's current term of service, all A points after the original loss of rally/sideout (after which the illegal service occurred) may be removed.

Plus don't forget to have the offending team put their players in the correct line up either, whether that means the whole team rotated wrong or only two players were out of order. Otherwise that means at the moment Team B makes serve contact, it would be an automatic point for Team B, while Team A has overlapped players being out of rotation still.
