In Utrip men's class E, this is possible, especially if two of the batters were offensive ejections for hitting over the fence homeruns.

I'll go with 2 in a half inning in ASA FP. DP with a BOO.

Two.
ASA SP:
B1...Intentional walk
B2...swings at 1st pitch and hits into a DP
B3...swings at 1st pitch and flies out.

3 outs. Two pitches thrown

ASA SP. Three intentional walks, and all three were BOO.

No pitches.
