We (South Carolina) were forwarded an email from the editor which specifically indicated that part (a) on page 32, 4.19.3E, should read "intentional foul due to excessive contact while the ball is live," which is the same as part (b).

Although depending on what defines "throwing to the floor," I may be going flagrant on this one.
