Lane Violations

[The_Rookie]

Here are 3 different scenarios:

1) Def. Player steps into lane b4 hitting rim and free throw is missed.

2) Def player steps in an then off. player steps in b4 hitting rim and free throw missed.

3) Def player steps in lane and shooter steps in lane b4 ball hits rim.

4) Def Player steps in lane and shooter shoots an air ball and misses rim.

What are your NFHS rulings under each scenario??
