Couple things that will give you the answer on your own for these questions.
Anytime the defense has a violation, the violation is ignored if the shot is made. If the shot is missed then you apply the violation if it is the first of a lane line violation. Anytime the offense commits a violation, then the play is dead and they lose the opportunity to complete the FT. That all being said we only enforce the first lane line violation and ignore the second. Any other violation could cause a double violation which includes anything by the shooter.
