As usual, Bob has nailed this one. Per the 2007-08 Case Book:
9.1.1 Situation: A1, at the free throw line to attempt a free throw....(b) accidentally drops the ball before the throwing motion is started. RULING: In...(b) the official should sound the whistle to prevent any violations and then start the free throw procedure again. No free-throw violation should be called in this situation.

Per 2008-09 Case Book:
In (b) a violation should be called on A1.

The ruling for a "muff" remains as it did in the 2007-08 Case Book. The administering official is to sound his/her whistle immediately -- BEFORE the shooter crosses the line -- in order to prevent a violation and then start the free throw procedure again.

A related question, the Rules Book and Officials Manual use the shaded text method to show changes in Rules/Procedures. Why is the Case Book not similarly shaded? It would be really nice to include a listing of Cases that are no longer valid in the back of the book as well.
