I agree you do not give them the ball for the attempt until you correct the problem. No reason to have a violation for this case.

I don't know. With adult league intramural basketball; tell them once. Call the violation after that. I'm not going to continue to hold up the game by warning them or asking them to move. Give the shooter a second chance one time, and they'll quit.
