Ohio -- The majority of the leagues/conferences uses 3 person crews for varsity boys regular season. There are very few that use 2 man crews. On the girls side the 3 person crew is not used in as many leagues/conferences as the boys but I would still say more use three than don't. Regular season fees vary throughout the state $40 - $65 for a varsity game.
Three person crews are used for both boys and girls in OHSAA Tournament play, Sectional>District >Regional>State. I think fees start at $50 with a little more at each level, capping at $70 for State games. At each level you get paid mileage. I cannot quote the mileage rate because each of the five Districts in the state pays a little different (i.e. .25 per mile round trip or .35 per mile one way). It is not until the Regional/State level that OHSAA pays a set mileage.
