I think the mindset is the defense should pick first since they have positional advantage.

That's my approach and how I've handled it the 2 or 3 times it has been a problem.

I would have thought the other way -- wouldn't you allow the defense the opporitunity to match up against the player that they want in this instance? That seems to be how McCutchen approached it, otherwise he would have told the Wizards player to pick a spot.

You are right, that's what I meant. I misread Adam's post. Defense will get the last choice.

Is there anything in any of the rule books about who gets to choose? (I know baseball addressed the ambidextrous pitcher vs. switch hitter "problem" by making a specific rule about who has to choose first.)

No (NFHS, NCAAW, and, I think, NCAAM. Don't know NBA).

I (would) go from lowest spot to highest spot.

... because the defense has to be there (unless they're down to only one player) in the lowest spot. The offensive player in the second spot doesn't have to be there at all.
