OBR DH question

DH in the 4 slot, batting for F1, Smith. While on Defense, Smith moves to RF. Does Smith now bat in the spot of the original RF?
Is the DH terminated at this point? And does the new F1 now bat in the old DH slot??

Second sit: DH enters the game to pitch. DH obviously stays in the same slot. original F1 moves to RF and then bats in the spot vacated by RF. DH is terminated?

First situation is correct. If F1 assumes another defensive position, he bats in the DH's slot, unless it is part of a multiple switch.

Second situation is also correct. The DH is always terminated when he takes a defensive position.

I think the manager can choose the batting order spots of Smith and the new pitcher -- one has to bat in the original DH spot and the other in the original right fielder's spot (assuming the substitutions were made at teh same time).

J/R goes on to say that the manager must announce his intentions to the umpire before signaling for a new pitcher. If he signals for a new pitcher without telling the umpire his intentions the pitcher has to leave the game. So "at the same time" is only after discussion with the umpire.
