Inadvertent Free Throw

So here is the situation. B21 picks up his 5th foul and the C official reports the foul to the table. Table notifies C that B21 has 5 fouls and C notifies head coach. As head coach is figuring out who to put into the game, lead official administers the basketball to A32 who shoots and makes the first free throw of two. B head coach replaces B21, and wants the free throw to not count as he had not properly replaced B21 prior to lead official administering the basketball. Crew decides that there was no harm and counts the free throw. What should have happened other than of course the lead official making sure everyone was a go for the free throws and not administering the basketball to A32.
