Batter hit by pitch while in front of box

Please cite rule and call for all rule sets. Forgive me, I know this has been posted here before but can't find it.

Batter hit with pitch in box. Dead ball - award first base. (ball does not belong in batters box, onus is on pitcher to not pitch it there.)

Batter hit with pitch while out of batter's box. Ruling?

Batter hit with pitch while in front of batter's box. I thought it was onus on batter, since she's now "closer to pitcher" and not where she's supposed to be. Dead ball, ball.

Can someone clarify? All rule sets please. (ASA, PONY, USSSA, NFHS, NCAA).

Okay. Left handed slap hitter. She was running up to hit pitch.

The scenario involved a slap hitter who got hit when she was outside the batter's box. I don't believe what you said applies in that scenario, which is what Mike was addressing.
