ball hits hands

Batter squares to bunt and ball hits the hand. Dead ball strike is the correct call.
Similar situation, but batter pulls back as they see the pitch is inside and the ball still hits the hand. Do you still have a DBStrike, or could you have a hit batter, and award them 1st base.

if out of the strike zone, it's a hit batter, award 1B (if an effort was made to avoid the pitch)

Not necessarily. Speaking ASA, if no effort to strike the ball was made, jmkupka's response applies here as well as the follow-up question.

I should have clarified for NF. They changed it back this year to, don't pull back and it is a strike.
