Go ahead and allow that - Case book play 8-11-6 alludes to that situation. 8.6.E prevents using a courtesy runner for the DP when it's the DEFO who is pitching or catching. When the DEFO leaves the game and the DP is the pitcher or catcher, it's OK to use a courtesy runner.
