I've had to request officiating resume input from my guys over here in the UK when there was a vacancy in the European Association.

Other than contact details (name adress, phone, E-mail etc) and maybe one line about your real life job, all we cared about was the officiating related stuff -
your age,
any playing / coaching experience,
brief details of other sports officiated,
football clinics attended,
any experience training new officials,
any assigning experience,
approx number of games worked,
preferred positions,
years of experience,
explanation of any gaps eg due to injury,
level of football worked,
big games worked, etc. etc.

Hope this helps....
