Evaluation Forms
Gentlemen,
I have searched and could not find anything here even though I know it's been brought up before. We are trying to do observations on a formal basis this year. Does anyone have a format or a form I can use that is good for high school observations? Any help is appreciated.

One of the best forms I have seen is from the SDHSAA. I has a separate form for each position, and a cumulative crew form. Contact Bob Lowery for the forms. His email can be found on this link. http://www.sdhsaa.com/general/staff.asp

Hmm, I thought I had one on my site. I have one the Missouri HS League uses that I typed up. I can post it or email it if you like. Let me know if you would like to see it. It is more universal and not position specific.
