It looks suspect but I can't quite see what he's actually holding or doing, so not enough information for me just on the video. As to the ineligible downfield, looks like the catch is made in the neutral zone, very close but enough for me to say no flag.

4 - possible, but not from this angle.
5 - clearly legal. Keep your eye on the spot where the ball was snapped and watch that spot until the ball is caught. Definitely legal and VERY well designed.

I thought it was caught beyond the LOS but very close. I probably wouldn't be able to tell without a replay.

I bet you could tell if you were a wing on the opposite side from the down box.

Unfortunately, there is an element of risk associated with really well designed and well executed "surprise" plays, when they work so well, they surprise EVERYBODY.

Maybe (hopefully) the Head Coach advised the Referee of the possibility of using this "special" play during their pre-game discussion, and the crew was ready for it and determined it was pulled off successfully.
