Rule Interpretnations - Substitutions and Warm-ups?

May I please ask for some rule interpretations and references on the following:

(1) A1 is at the scorer's table waiting for a substitution. A dead ball scenario occurs and the official looks to beckon the player into the game. Coach of Team A states he does not want a substitution. This is a legal play. RULING: The substitution DOES NOT have to enter the game and A1 may return to the bench.

(2) During pre-game warm-ups, Team A has not come out to the floor ( during the 15 minute pre-game warm up time - NFHS ) Team B ia now taking their warm-ups on BOTH sides of the court. Are there any rules that stipulate that Team A/B must ONLY warm up on their end of the court?

(1) Legal. Similar situation in Casebook 3.3.1 C

(2) Teams must use their own half of the court. Source: 2010-11 Points of Emphasis under Sporting Behavior p.66. Not sure if it's specifically stated in the rules; but 4-5-2 seems to indicate it.

Thanks for posting, BillyMac. Got home late from a tournament game last night and didn't feel like getting out the scissors and Elmers.
