First, what Adam said below. But then if it has to be a call: Who encroached on whose legally acquired space?

We are seeing this too in our neck of the woods. If the kick is severe enough to hurt player is it a personal foul or a player control foul?

He is an airborne shooter, right? That should answer your question.

A player control foul is a personal foul.

So on the follow-thru if a players hand and arms extend out past the vertical plane but a player runs by and hits his arm while he's airborne, this is a foul. But if it's the foot or leg, we just let it go?

If contact is marginal, then go with a no call...if it isn't, then send it the other way.

The NBA actually made this a point of emphasis the year, I believe. They want more officials calling offensive fouls on players for creating contact with the leg kick on jump shots.

I have seen it called a few times.

In my HS school games I have not seen this much to the extent that a PC was warranted but I have seen a few flops with extending the leg and flopping to make it look like a foul on the defender.

Yes, contact on the jump shot is a POE this year...including the shooter creating contact by extending his leg. In fact, I believe the league has been trying to crack down on this the past couple of years, and I've definitely seen more no-call or offensive fouls call on these type of plays.
