$40 per game in my part of Kansas, no mileage, all doubleheaders. $45 in Missouri, mileage, mostly doubleheaders.

In the Chicago area, I think it's $47 for lower level, and $57 for Varsity. Some (maybe even most, but not all) lower level games are two games, but I don't know the rate. No mileage (but many games are "close" mileage-wise, but long time wise -- it took me 30 minutes to get to a game 6 miles away last night).
The problem is that is conference related or NISOC (a cartel) related. I know many many private schools pay more varsity and lower level single games. I have received $65 for a varsity contest recently. And if you get outside of Chicago that fee is certainly higher in my experience as well.

Peace

That is being proposed in Texas for metro areas and will probably be implemented if the lawsuit is ever resolved. Though the zones are figured from a center point in the metro area (downtown Houston in my case). Mileage will be based upon how far schools are from the center point based upon three separate zones.

Oh and sub-varsity scheduling fees here are $1.25/game for hs and ms games. No idea what varsity fees are.

JV 3 person = 28 per official plus 14 for mileage. 10% in assigning fees on the game fee only.

JV 2 person = 38 per and the same mileage and assigning fees.

V 3person is 49 per with same mileage and fees.

Colorado pays a $2 mileage stipend for metro schools. Travel games (outside the metro area) pay mileage to one driver for each set of officials.

Middle school $40 (usually, there are two)
JV $46
Varsity $62.50
In all cases, plus mileage.

Locations of games can vary greatly. Last night, I worked in my hometown (exhibition games, all officials were either townies and/or worked for the district). Tomorrow's assignment is a mere three miles from my office. On the other end, a have a Saturday morning pair an hour, 15 minutes away.

Almost all of ours is double headers.
2-JH = $80
2-FR = $90
1 jv & 1 var = $100
2 Var = $110
Plus $0.40 per mile round trip paid to one official.

I have some games that are almost 90 miles one way.

I forgot to add that we tack on $15 to any game fee when only one game will be played. I have a game Saturday an hour and 45 minutes from my house and it's a single game, so I'll make $70. Single games are awesome when they're close to home, and they're awful when they're almost two hours away.

North Iowa
$30/per for lower level 8th Under
JV - $50-60 - Single game - 2 man
Varsity DH Girl/Boy - $80-95 - 3 man
Rare mileage - bigger conferences - one person buys on way home.

Massachusetts:

55 - Subvarsity (which includes middle school)
77 - Varsity

No mileage, almost always 2-man. If the school opts for a 3-man varsity crew, I believe the pay is $68.

Distance of games depends on what county you live in and what board you're on. For my board, all games are within 25 miles or so, and usually less.
