Yes. You'll see it in men's D-1 games from time to time. I remember Hightower being one of the two remaining -- it could've been last year.

They really struggled to get up and down the 94-foot floor. And they rotated ball side the exact same way as if they were working 3-person -- they just went back across when they went the other direction.

Mostly, they looked a bit awkward. Which I get since they never do 2-person anymore.

I think that was when Wellmer went down at Michigan State.

Sure...it happens infrequently at the NBA and NCAA level. Heck, even saw an NFL recently where they had to go to 6-man mechanics due to the umpire being injured.

Anyhow, they just go to their prescribed two-man mechanics. Most games aren't going to be stopped just because an official is injured.

Yes there was a game where Welmer went down with a knee injury and Hightower and an an official in my area worked who I know worked with them. It was the Kansas at Michigan State Game. Both coaches were all over him the rest of the game and left Hightower alone.

I think there was an NBA game that was officiated with 2 whistles a few years back in Boston... IIRC, the 3rd official's flight was canceled due to a big snowstorm so he couldn't make it into town.
