If the book isn't ready, I talk to the timer. If the timer isn't there, I'll grab the ball and take my traditional 3 pounds of air out of it. If the ball isn't there, I'll test the AP arrow. I take the rules seriously, but we simply do not have problems with this. 1 game in 25 might have the situation where the book isn't ready.
