You're probably right. They may not even know you can violate during a T shot.

Probably thought the same officials that were so bent on sticking an administrative T here, might view whistling the free thrower stepping over the line as a game interruptor.

Good judgement by the coach and free thrower.

I would have called the T, and suggested a violation to the coach after he said he didn't want it.

ESPN ran a segment on this on SportsCenter. They even interviewed one of the officials. He stated that he had to assess the technical foul, despite the opposing coach stating that he didn't desire it, so as not to compromise the integrity of the game.

I happen to agree with him. The official's job is to make the proper call. It is up to the coaches and players to decide what they wish to do with their opportunities.
