Shouldn't the non-administering official in a 2-man crew be positioned to watch the shooter just like any normal free throw while also putting her/himself in a position to monitor all the non-shooters?

Yes, I'm talking about a 2 man crew. And in this specific case, I suggest disregarding the usual free throw mechanics. There is nobody on the lane for the lead to watch. Let him watch the shooter. If he cares deeply about shooter's feet, he can even move to where he can see them more clearly. However, I think he's better off devoting most of his attention to the other 9 players, and not worrying too much about the shooter.
As for the trail: Depending on what caused the T, it is very much in his best interest to closely watch the non-shooters. They've got time on their hands, and they're all in close proximity. That's never a fantastic combination. And if there's been very recent trouble...
Definitely non-standard free throw mechanics. But better suited to this unique situation, I feel.
