Keep a list of who is in the game, and then who subs in / out.

So, if the starters are 1 though 5, the list is:

1
2
3
4
5

If 11 and 12 enter for 3 and 4, the list is:

1
2
X, 11
X, 12
5

If 3 now enters for 1, the list is:

X, 3
2
X, 11
X, 12
5

If after a TO, those aren't the 5 on the floor, hit the horn.
