I've been a regular reader on this site for a few years and find it very helpful, interesting and entertaining. I thought I'd finally register and join in the fun. So here is my first question.

At the beginning of the second half (high school var game) Team A Asst. coach reports starters to the scorer’s table. Second half starts, first dead ball we get the horn from the table, one of the players for team A was not reported as a starter for the second half. What do we have?

I say nothing; once the second half started the players all became legal. I can't find any rules references.

You are correct. 10-2 Penalty is the reference which states that it is too late to penalize this. In your case it would have had to have been caught before the ball becomes live to start the 3rd quarter.
