I dont think that someone is "responsible" -- I think he was just looking for extra help (tv replay or something). It might be tracked, but it's not their official job. But, until one of us works the plate in MLB, or has the "job" in the press box, we won't know for sure.

Ron Darling mentions that once in a while, the pitcher will forget what type of pitch to throw after given the signal.

I don't think so.

People know the OS does so he's the one to call rather than polling the participants.
