Catcher steps out of the catcher's box on the first pitch of the game meant to be the first pitch of an intentional walk.

You just announce them in FED (and in OBR now). No pitches get thrown.

No one calls that anyhow.

The coach doesn't have to announce the intentional walk. He still has the option of throwing the 4 pitches. I'm not sure why he would want to risk it, but it can be done.

Is that really a balk if nobody is on base?

It is an illega pitch and the batter is awarded a ball.

Another rules difference. It's not an IP in OBR. An illegal pitch in OBR is only a quick pitch or a pitch while not in contact. In OBR it's a balk (never called) when there are runners on.

I get what you're saying. Do the requested IBB, then balk before a pitch is thrown.

Hmm...? Good thinking. Can't. If I read you right.

ball is dead during IBB. At least in FED. Don't know how OBR will interpret.

It's live after. You know, when the next batter is up.
