intentional USC by pitcher

To have the team change pitchers in the middle of an at-bat, have you ever known of a pitcher to commit unsportsmanlike conduct to try to get disqualified? Are there any rules or practices you know of to prevent such tactical fouling?

No. No.

In FED, of course, the pitcher can be changed at any time assuming he's faced one hitter, of course).

In OBR, the manager is (generally) going to continue to "believe" in his choice to leave the pitcher in the game if he had a visit or a change (and if there was no visti, then the pitcher can be changed in the middle of an at bat).

And, if you really want the pitcher out, it's easier to feign an injury. Any "unsporting" aspect to that will be dealt with by the league.
