Written warnings

This year in games played under National Federation rules, for any minor violation of Rule 3-3-1f, 1-5, (g-k), the offending coach will receive a written warning and will be restricted to the bench/dugout for the remainder of the game. I would be interested in knowing if any umpire, any umpire board, or any state association has yet designed a written warning. If so, would you be willing to share its verbiage with the rest of us? I envision it to be short and sweet, containing the date of the game, the coach's name, the reason for the restriction, and the issuing umpire's name. Myself, I would prefer the form be in duplicate, so that I could save a copy for my records. What are your thoughts?
