contagious disease form

Rule 4.2.3 states that the form needs to be current. I seem to recall current meant that the form had to be filled out within a certain number of days before competition.
I had a dual last night where the area in question was covering the entire inside part of the elbow and it was over 6" long and was the width of the arm.
The coach produced the proper form but it was dated 11/19/07. Doesn't this form have to be updated so many days before competition?

In Ohio I am not familiar with any time limit on a form other than what is printed, but IMO if it looks bad to you, don't let him wrestle. It sounds like he needs more treatment on the arm to me.
