Unusual rotation fault

At my last convention center tournament, the teams came out to start the match and the R2 checked the line-up sheets. The team to my left is receiving the serve and the setter started in position 1. (So the player is on the opposite side of the court from her bench.)

As I beckon for serve, the receiving coach calls the setter over for -- I guess -- last-second instructions. She's now standing next to the coach, which puts her in position 5. But I've already beckoned. The serving player actually waited for a beat like she realized something was wrong. But then she served.

Never had that happen before at any point in a match, let alone the very first point.

I hope the R2 recognized the illegal alignment and made the call?

She did. It was the coach of the work team, as opposed to a player. So she knew.

Had one once where the team started in receive rotation 1. Then won the point but filed to rotate -- so the setter served in position 1. After I called the fault, the coach just shook her head.

A few weeks ago at a men's tournament I had an illegal block by...someone who wasn't the setter. And it wasn't the libero attempting to block, nor was it someone who forgot they were back row. And it was a 7:30am match, so it took an extra second to process.
