Unreported Subsituite
Batter who is supposed to bat was hurt the preceding inning. Replacement for the injured player comes to bat, the first pitch is a strike, the opposing coach calls time and say the opposing coach did not report the subsitute. What do you call.

In this case speaking ASA; Player is disqualified and all action stands. You would need a sub or re-entry of injured player if they have re-entry possibility's and they would assume the count. If no sub then an out and short-handed, if they aren't already playing short-handed. If they're playing short-handed that's a ball game. All this is if your league doesn't have their own adopted rules. 4-6-C

Without my rule books to look at (they are packed as I am moving), NFHS would have this as a warning to the offending coach. Next offense would be a player and coach DQ, if I am not mistaken.
