A foul that occurs after a fumble and before possession is (re)established: the basic spot is the end of the run, and any foul that occurs during the loose ball will be penalized from the basic spot, unless you need to apply the all-but-one principle. ;o)

As already mentioned, this may be an NCAA ruling, as in NF ball, a fumble in or behind the NZ is a loose ball play, and a fumble that occurs downfield is part of a running play even though the ball is loose. Finally, as I already said before, (almost) every live-ball foul is enforced using the all-but-one principle.

REPLY: I think sir_eldren might be confusing a "loose ball" with a "loose ball play." In NCAA rules, there's no such thing as a "loose ball play" for the purposes of penalty enforcement, so he might just be referring to a play situation where the ball is loose. Is that it, sir_eldren?
