Charting Fouls

Anybody have any form examples of charting fouls/whistles? Blank or completed would be ok. Thanks in advance.

I have some spreadsheets which Freddy shared with me a couple years ago when I was starting. I would love to share them with you but would need Freddy's permission first. Freddy?

I only copyright things I think I can make money on, which I never do.
I probably got those spreadsheets from someone else . . . share away.

When looking for the "Just Right" charting form, keep in mind that what you choose to identify, document, and track may well be specific to what you'd best be targeting. The standard "call good, call not good, no call good, no call not good" might be all a veteran crew needs to know, according to some. However, a newer crew might benefit from designations like "foul call in PCA", "foul call out of PCA", foul by T or L or C and by whom, "marginal" foul vs. "solid" foul, etc. There may not be a "one size fits all" that benefits everyone. I'd advise what others have advised me previously--identify what you want to target and make up your own spreadsheet, maybe based on the ideas you glean from others.
